Employee Authorization and Release Form
Employee Authorization and Release allows KAAB Continuum LLC to conduct necessary required background checks.
By affixing my signature, I authorize KAAB Continuum LLC to conduct investigation of my employment, Education, and activities. I Understand this may include a criminal background check, police and motor vehicle investigation, as well as professional inquiries. I understand that the results of this investigation may be used to make decisions regarding my employment.
I voluntarily consent to a random drug screening test which may include urine, blood, or breath sample. I consent to the release of the test results to the management of KAAB Continuum LLC.
I hereby authorize KAAB Continuum LLC to release any and all information contained in my employment file to any facility or entity with whom KAAB Continuum LLC has obligations to or entered in a contract with. The decision to release any and all information will be made solely by KAAB Continuum LLC. Such information will be provided only to authorized representatives of appropriate entities. For all other purposes, my employment records shall be kept confidential by KAAB Continuum LLC.
I hereby release KAAB Continuum LLC, its personnel, and affiliates from any liability from or in connection with any undertakings mentioned above.
